Advanced Practice Provider - Including Clinic and Call

SUMMARY:
Under general supervision, provides independent acute and/or routine medical care in both in-patient and out-patient
clinical settings.

ESSENTIAL FUNCTIONS:

  • On Call
  • Will round and create notes on all admitted post-surgical hospital patients and communicate
    with attending urologist on-call regarding clinical status, outstanding tests/results, and plan; all
    notes should be done before noon unless there are extenuating circumstances.

  • Will discharge admitted post-surgical patients and document discharge summaries.
  • Will evaluate, examine and document a new consult on all new inpatient or ER consultations
    that come in during his or her on-call shift.

  • May be required to attempt some bedside procedures on inpatient consultations, when
    indicated, including complicated Foley catheter placement, bladder irrigation, penile
    injection/irrigation/aspiration.

  • Will be responsible for posting urgent and emergent cases to be done on inpatient consultations
    to the OR schedule as needed.

  • On-call APP will ensure that in-patient consultation patient who are scheduled for
    surgery/procedure have a completed informed consent form in the patient chart

  • At the end of the on-call shift, the on-call APP will have a phone conversation with the on-call
    urologist to update the status of active inpatients, and should also sign out with the next day on-
    call APP if applicable

  • Will maintain the on-call patient list in Teams and ensure the list is updated prior to sign out
  • Will be the first point of contact for outpatient clinic calls to the on-call provider, and will
    document an appropriate phone note in Athena/CPS

Clinic

  • Assess patient health status including state of wellness, compliance with care plan and
    determines appropriate diagnostic and therapeutic procedures.

    therapies. Will be directly involved in general urology treatment, as well as, specialized urologic

  • Prepares equipment and assists physician or APP during examination, procedures and testing of
    patients.

  • Maintains timely flow of patients.
  • Responds to and refers incoming telephone calls. Instructs patient and family regarding medications
    and treatment instructions per orders of the physician or APP.

  • Responsible for conducting all lab result call backs to patients as directed by the physician and APP;
    appropriately document in patient chart.

  • Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Assists suite
    clinical staff with stocking and ordering of medical supplies.

  • Assist with minor procedures in clinic that can be performed in the exam rooms.
  • Maintains patient confidentiality.
  • Assist with weekly PSA testing.
  • Answers telephone, screens calls, takes messages, and provides information.
  • Provides support services to patients and medical staff. Answer questions regarding patient
    appointments and testing.

  • Appropriately enter patient information into patient medical record.
  • Schedules, coordinates and reschedules patient appointments. Relays necessary messages to staff
    and patients.

  • Attends staff meetings.

  • Collaborate and plan with Radiologist, referring physicians for the selection of procedural protocols.
  • Performs radiographic and fluoroscopic exams utilizing the CR and DR GE machine as ordered by the
    Urologist, PA-C, NP and Radiologist.
  • Starts IVs and draws labs to screen for contrast safety.
  • Preps all charts for the PACS system utilizing the EMR available.
  • Uses a variety of radiation protection and shielding materials.
  • Prepares contrast media and barium. Performs intravenous contrast injections including the use of
    automatic injectors.
  • Ensures equipment is in working order. Reports equipment malfunctions to the Ancillary Services
    Manager.
  • Logs radiologic procedures completed. Processes related paperwork using computer equipment as
    directed.
  • Maintains examination rooms. Stocks necessary medical and radiologic supplies.
  • Develops and processes radiologic exams utilizing PACS and EMR for Radiologist interpretation.
  • Document and communicate patient related information thoroughly and accurately.
  • Demonstrate current competencies for job position.

    post-op management in coordination with MD over-site.

  • Responsible for conducting all lab result call backs to patients as directed by the physician and
    physician extenders; appropriately document in patient chart.

  • Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Suite team
    conducts stocking and ordering of medical supplies.

  • Assist with minor procedures in clinic that can be performed in the exam rooms.

    (utilizing clinical EMR phone notes and flag system) and timely.

  • Utilized Tiger Text for sending encrypted text messages to physicians and other staff set up on Tiger
    Text.

  • Updates demographic information in computer as appropriate.
  • Assists other departments as directed (i.e., processing recalls, mailings, etc.).
  • Maintain the Phone bank so it is clean and organized.
    Keeps records of phone bank function as directed.

  • May assist with some scheduling and rescheduling as time permits.

PROFESSIONAL REQUIREMENTS:

    and clinics. Relays necessary information to staff and patients.

  • Assists patients with ambulatory difficulties.
  • Answers telephone, screen calls, takes messages, and provides information.
  • Obtain the necessary prior-authorization for cystos as needed.
  • Back-up to other support staff positions as requested.
  • Screens visitors and responds to routine requests for information.

Radiation Therapist

ESSENTIAL FUNCTIONS:
 
  • Responsible for administering radiation to patients under direction of a Radiation Oncologist.
  • Coordinate patient and work flow for all external beam radiation therapy.
  • Communicate with Ancillary Services Manager on a regular basis with recommendations to improve
    patient care and staff development.

  • Performs tumor localization procedures utilizing IGRT, patient immobilization, ultrasound, etc.
  • Delivers accurately the prescribed planned course of radiation therapy.
  • Observes patients for unusual reactions and events.
  • Performs daily linear accelerator warm-up procedures and daily QA.
  • Operate linear accelerator by setting appropriate controls on console and activating the machine to
    deliver prescribed treatment indicated on the treatment plan while monitoring patient and equipment
    during treatment.

  • Coordinate with Dosimetrist and or Physicist in technical and clinical issues.
  • Accurately schedule patients for daily treatments and filming.
  • Accurately bill patients for daily treatments and filming.
  • Accurately performs quality checks for treatment accuracy.
  • Coordinates patient and work flow for all treatment plans.
  • Perform QA procedures on simulator.
  • Accountable in simulation for positioning the patient in proper orientation using positioning aids and
    immobilization devices; work with physician to obtain optimum fields of treatment using CT. Obtain
    simulation films; take photos of simulation fields, tattoo patient and transfer set up instructions into
    EMR.

  • Maintain cleanliness of the equipment, treatment room, simulation room and consoles and workstation
  • Demonstrate expertise in care and the precise delivery of the prescribed dose of radiation to patients
    undergoing radiation therapy.

  • Demonstrate expertise in all aspects of radiation treatments and simulation.
  • Performs administrative tasks and other duties as assigned.

ESSENTIAL FUNCTIONS:
 
  • Responsible for administering radiation to patients under direction of a Radiation Oncologist.
  • Coordinate patient and work flow for all external beam radiation therapy.
  • Communicate with Ancillary Services Manager on a regular basis with recommendations to improve
    patient care and staff development.

  • Performs tumor localization procedures utilizing IGRT, patient immobilization, ultrasound, etc.
  • Delivers accurately the prescribed planned course of radiation therapy.
  • Observes patients for unusual reactions and events.
  • Performs daily linear accelerator warm-up procedures and daily QA.
  • Operate linear accelerator by setting appropriate controls on console and activating the machine to
    deliver prescribed treatment indicated on the treatment plan while monitoring patient and equipment
    during treatment.

  • Coordinate with Dosimetrist and or Physicist in technical and clinical issues.
  • Accurately schedule patients for daily treatments and filming.
  • Accurately bill patients for daily treatments and filming.
  • Accurately performs quality checks for treatment accuracy.
  • Coordinates patient and work flow for all treatment plans.
  • Perform QA procedures on simulator.
  • Accountable in simulation for positioning the patient in proper orientation using positioning aids and
    immobilization devices; work with physician to obtain optimum fields of treatment using CT. Obtain
    simulation films; take photos of simulation fields, tattoo patient and transfer set up instructions into
    EMR.

  • Maintain cleanliness of the equipment, treatment room, simulation room and consoles and workstation
  • Demonstrate expertise in care and the precise delivery of the prescribed dose of radiation to patients
    undergoing radiation therapy.

  • Demonstrate expertise in all aspects of radiation treatments and simulation.
  • Performs administrative tasks and other duties as assigned.

Registered Nurse

ESSENTIAL FUNCTIONS: 

  • Provides professional nursing care to a specified group of patients in infusion, clinic and procedural
    settings caring for non-hospitalized patients.

  • Notifies physician of abnormal findings.
  • Performs telephone triage for New Patients to The Urology Center of Colorado.
  • Establishes a care plan in collaboration with the patient, family and health care team.
  • Coordinates ongoing care with specialized diagnostic and therapeutic procedures.
  • Administers medications and treatments as ordered.
  • Independently manages IV and port access for patients in all areas of the center, – CT, Lab, Radiation
    Oncology, and Clinic.

  • Responsible for patient assessment continuously during conscious sedation.
  • Responsible for pre-op teaching, reviewing pre-op labs, ECG and ensuring cardiac clearance. Provides
    post-op management in coordination with MD over-site.

  • Provides health education to patients and families.
  • Initiates CPR and other emergency measures if needed in the clinic environment. Assists in patient
    transfer to tertiary center when needed.

  • Covers other nursing support roles with the center.
  • Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Prepares list of
    medical supplies needed. Prepares equipment and aids physician during treatment, examination and
    testing of patients.

  • Participates in quality initiatives, process improvement and research activities to improve patient
    outcomes, patient experience and patient safety.

  • Maintains professional growth and development in the nursing field.
  • Supports compliance with all regulatory requirement and standards.
  • Maintains/reviews patient records, charts and other pertinent information.

  • Observes, records and reports patient’s condition and reaction to drugs and treatments to
    physicians.

  • Dispenses medication as directed (within scope of license)
  • Educates patient/family about diagnostic procedures, medications as directed, nutrition and
    maintenance of health and wellness.
